As Yaşam Diyet (hereinafter referred to as the "Company"), acting in the capacity of data controller pursuant to the Turkish Personal Data Protection Law No. 6698 (KVKK), we would like to inform you about the processing of your personal data as described below.
Your personal data are processed for the purposes of providing dietitian services, tracking appointments, preparing nutrition programs, measuring client satisfaction, assessing your health status, carrying out payment transactions, and fulfilling legal obligations.
Your identity information (name, surname, Turkish Republic ID number), contact information (telephone, e-mail, address), health information (height, weight, body analysis, health history, nutritional habits), payment information, and appointment history are processed.
Your personal data are collected through face-to-face meetings, telephone calls, e-mail, our website, WhatsApp, and other communication channels, based on the legal grounds set out in Articles 5 and 6 of the Turkish Personal Data Protection Law No. 6698 (KVKK).
Your personal data may be transferred, in accordance with Articles 8 and 9 of the Turkish Personal Data Protection Law No. 6698 (KVKK), to official institutions and organizations for the fulfilment of legal obligations, to cooperating laboratories and healthcare institutions for the coordination of healthcare services, to banks for payment transactions, and to attorneys from whom legal consultancy services are obtained.
Pursuant to Article 11 of the Turkish Personal Data Protection Law No. 6698 (KVKK), with regard to your personal data, you have the right to learn whether your data are being processed; to learn the purpose of processing and whether they are used in accordance with that purpose; to request information about transfers domestically and abroad; to request the rectification of incomplete or inaccurately processed data; to request the erasure or destruction of data; to request that third parties to whom the data have been transferred be notified accordingly; to object to the emergence of a result against you through the analysis of processed data exclusively by automated systems; and to request compensation for damages arising from unlawful data processing.
